Instructor Orders Instructor orders cannot be taken over the phone AND you must place your orders TWO WEEKS, NO EXCEPTION in advance of when you schedule your class. To place an order, access the online Instructor Request Form. Orders must be submitted on-line, mailed to the Chapter House, or dropped off at the Health & Safety Department during regular business hours (9 a.m. - 3 p.m.).
The on-line application will require that you have an email account set up on your computer using a program like Microsoft Outlook or Outlook Express, or you can copy the whole page, and paste it into your email.
